Uit de NOS uitzending Onthulling Nationaal Holocaust Namenmonument op 19 september 2021.
Een uitsnede van een lijst met daarop een aantal gevangenen van Auschwitz met daarop hun herkomst, naam, geboortedatum en gevangenenummer.
Alle gevangenen op deze uitsnede zijn op 12 augustus 1942 vermoord, waarschijnlijk is het dus een kopie uit een Sterbebuch.

Volgens onderzoek door het Nederlandse Rode Kruis is Herman op 16 juli 1942 op transport gesteld van kamp Amersfoort naar Auschwitz. Op deze kaart staat ook zijn gevangenenummer genoteerd, het nummer dat hij bij aankomst in Auschwitz kreeg en zijn overlijdensdatum in Auschwitz op 12/13 augustus 1942.
