Hartog Polak was a son of Benjamin Moses Polak and Rachel Aandagt. He married Vrouwtje Zwaaf in 1888 in Amsterdam. She passed away in 1910. They had twelve children: Rachel, Klaartje (who died at a young age), Sophia, Benjamin, Israël, Abraham, Betje, Engelina (who died at a young age), Hadassa, Samuel (who died at a young age), Jacob and Helena.
