Levie Polak was a son of Abraham Meijer Polak and Aaltje Marcus Boekbinder. He was married on 21 December 1884 in Amsterdam to Grietje Slap, who passed away on 12 February 1939 and was interred in the Jewisch Cemetery of Diemen.
The couple had seven children, of whom Israel has died in childhood in 1902. The other six children, Klaartje, Hijman, Rebecca, Abraham, Esther and Meijer have been all killed during the Holocaust;.
