Levi Moses Polak was one of the two sons of Moses Polak (1867-1930) and Clara Rosenbaum. After becoming a widower, his father remarried, to Sara van Rooijen (Amsterdam, 3 augustus 1874 - Sobibor, 5 maart 1943). His parents and brother David (b. 23-7-1904) owned a secondhand shop in Deventer. After his father passed away, Sara Polak-van Rooijen moved to Amsterdam.
